Most major insurance companies cover the LAP-BAND® procedure as long as you meet their requirements. Your insurance may consider you a canidate for the LAP-BAND®, if your BMI is at least 35 (with two underlying health conditions) or a BMI of 40 or greater (with no underlying health conditions). Payment Plan
 

If you do not have insurance, do not meet your insurance requirements, or you have an exclusion on your policy, you can take advantage of our easy payment plans.
